Training Injury / Illness Incident Report

At the direction of the municipal police training council, this form is to be completed by the course director of a police training course when a training related injury or illness which causes a disruption in training for the affected student has occurred. This form is only required for instances which result in a full or partial disruption in training. The information in this form will be used to study the types and frequency of injuries common police basic and in-service training for a two year period.

This form is to be filed for courses that begin after September 1, 2006 and begin before September 1, 2008. It is the responsibility of the course director to complete this form after an injury or illness occurs. Once the training program has been completed, this form is to be submitted to the Division of Criminal Justice Services, Office of Public Safety along with the course completion documentation.
